BACKGROUND: Food and beverage promotion is a contributor to children's dietary behaviours, and ultimately, downstream health consequences. Broadcast television remains an important source of such advertising. The objective of this study was to examine and compare children and adolescent's exposure to food advertising on television in Canada over an entire year in a self-regulatory environment. METHODS: Television advertising data for 57 selected food and beverage categories were licensed from Numerator for 36 stations in Toronto, for 2019. The estimated average number of advertisements viewed by children aged 2-11 and adolescents aged 12-17 was determined overall, by food category, and by marketing technique. The healthfulness of advertisements was also assessed using Health Canada's Nutrient Profile Model. RESULTS: Overall in 2019, children viewed 2234.4 food ads/person/yr while adolescents viewed 1631.7 ads, exposure for both groups stemmed primarily from stations with general appeal, and both age groups were exposed to a range of powerful marketing techniques. Exposure to advertising for restaurants, snacks, breakfast food and candy and chocolate was high among both age groups and the healthfulness of most advertised products was considered poor. Adolescents were exposed to 36.4% more food products classified as unhealthy, had higher exposure to all marketing techniques examined, and were exposed to substantially more child-related marketing techniques compared to children. CONCLUSION: Children and adolescents were heavily exposed to food advertisements on television in 2019. Despite current self-regulatory policies, children's exposure to unhealthy food and beverages remains high. Differences in exposure to food advertisements by food category and healthfulness may suggest that adolescents are being disproportionately targeted by food companies as a result of self-regulatory marketing restrictions.

Importance: Celebrity social media posts engage millions of young followers daily, but the nutritional quality of foods and beverages in such posts, sponsored and unsponsored, is unknown. Objective: To quantify the nutritional quality of foods and beverages depicted in social media accounts of highly followed celebrities and assess whether nutritional quality is associated with post sponsorship, celebrity profession or gender, and followers' likes and comments. Design, Setting, and Participants: This cross-sectional study analyzed the content of food- and beverage-containing posts from Instagram (a photo- and video-sharing social media platform) accounts of 181 highly followed athletes, actors, actresses, television personalities, and music artists. Data were collected from May 2019 to March 2020. Main Outcomes and Measures: The nutritional quality of foods and beverages posted in celebrity social media accounts was rated using the Nutrient Profile Index (NPI) based on the sugar, sodium, energy, saturated fat, fiber, protein, and fruit and/or vegetable content per 100-g sample (a score of 0 indicated least healthy and 100, healthiest); foods with scores less than 64 and beverages with scores less than 70 were rated as "less healthy." Secondary outcomes were whether the nutritional quality of foods and beverages in social media posts was associated with post sponsorship, celebrity profession or gender, and followers' likes and comments. Mixed-effects regression models were used to estimate how outcomes differed across fixed effects. Results: The sample included social media accounts of 181 celebrities (66 actors, actresses, and television personalities [36.5%]; 64 music artists [35.4%]; and 51 athletes [28.2%]). A total of 102 celebrities (56.4%) were male, and the median age was 32 years (range, 17-73 years). Among 3065 social media posts containing 5180 total foods and beverages (2467 foods [47.6%]; 2713 beverages [52.4%]), snacks and sweets (920 [37.3%] of the foods) and alcoholic beverages (1375 [50.7%] of the beverages) were most common. Overall, 158 celebrity social media accounts (87.3%) earned a less healthy overall food nutrition score and 162 (89.5%) earned a less healthy overall beverage nutrition score, which would be unhealthy enough to fail legal youth advertising limits in the UK. For foods, social media posts with healthier nutrition scores were associated with significantly fewer likes (b, -0.003; 95% CI, -0.006 to 0.000; P = .04) and comments (b, -0.006; 95% CI, -0.009 to -0.003; P < .001) from followers. For beverages, nutrition scores were not significantly associated with likes (b, -0.010; 95% CI, -0.025 to 0.005; P = .18) or comments (b, -0.003; 95% CI, -0.022 to 0.016; P = .73). Only 147 food- or beverage-containing posts (4.8%) were sponsored by food- or beverage-relevant companies. Beverages in sponsored posts contained more than twice as much alcohol as those in nonsponsored posts (10.8 g [95% CI, 9.3 g to 12.3 g] per 100 g of beverage vs 5.3 g [95% CI, 4.7 g to 5.9 g] per 100 g of beverage). Conclusions and Relevance: In this cross-sectional study, most highly followed celebrity social media accounts depicted an unhealthy profile of foods and beverages, primarily in nonsponsored posts. These results suggest that influential depictions of unhealthy food and beverage consumption on social media may be a sociocultural problem that extends beyond advertisements and sponsorships, reinforcing unhealthy consumption norms.

BACKGROUND: Improving beverage patterns of children and adolescents is recommended for combatting obesity and reducing disease risk. Therefore, it is important to assess beverage intake quality in this population. For adults, the Healthy Beverage Index (HBI) was created to assess beverage intake quality, but a similar tool did not exist for children and adolescents. OBJECTIVE: The objective was to develop an HBI for US Children and Adolescents (HBI-CA), and then assess the validity and reliability of this tool. DESIGN: Modeled after the adult HBI, age-specific, evidence-based beverage recommendations were compiled. Ten components were included to assess beverage intake quality. Validity and reliability were assessed using cross-sectional data and methods similar to those used for the evaluation of the Healthy Eating Index. PARTICIPANTS: The 2015-2016 National Health and Nutrition Examination Survey provided 24-hour dietary recall data for 2,874 children and adolescents aged 2 to 19 years. MAIN OUTCOME MEASURES: HBI-CA scores were the main outcome measure. STATISTICAL ANALYSES PERFORMED: To assess validity, independent t tests were used to determine differences in HBI-CA component and total scores among groups, and principal component analysis was completed to examine multidimensionality of the HBI-CA. Pearson bivariate correlations were used to assess reliability. RESULTS: The HBI-CA produced a (mean ± standard error) total score of 69.2 ± 0.8, which is similar to the adult HBI mean total score of 63. Principal component analysis identified six factors, indicating the multidimensionality of the HBI-CA, with more than one combination of components contributing to variation in total scores. Most HBI-CA components were significantly correlated to the total score, with met fluid requirements, total beverage energy, sugar-sweetened beverage, and water components demonstrating the strongest correlations (r range = 0.335-0.735; P ≤ 0.01). CONCLUSIONS: The results provide preliminary evidence to support the validity and reliability of the HBI-CA. If future research establishes the predictive validity and sensitivity of the HBI-CA, this tool could be useful to quantify the beverage intake quality of children and adolescents.

(1) Background: This study aims to find the sugar content of market beverages and estimate the sugar intake from beverages among students in Beijing. (2) Methods: Using snapshotting, we collected the sugar content of beverages through their packages or nutrition labels. Combined with the statistic of student beverage consumption, we estimated students' sugar intake. (3) Results: The median sugar content of total beverages was 9.0 g/100 mL, among which the fruits/vegetable juices and beverages had the highest sugar content (10.0 g/100 mL). Sugar content in most beverages in Beijing was generally higher than the recommendations, and fruit/vegetable juices and beverages exceeded the most. The median of sugar intake from beverages among students was 5.3 g/d, and the main sources were fruit/vegetable juices and beverages, protein beverages and carbonated beverages. Sugar intake from beverages differed according to gender, age and living area. Higher sugar intake was found among boys, older students and rural students. (4) Conclusions: Sugar content in market beverages in Beijing were high. Gender, age and residence were the influencing factors of sugar intake. Targeted measures should be taken to decrease the sugar content in beverages, especially the fruit/vegetable juices and beverages and the sugar intake among students.

Studies have investigated the associations of coffee and tea with mammographic breast density (MBD) in premenopausal women with inconsistent results. We analyzed data from 375 premenopausal women who attended a screening mammogram at Washington University School of Medicine, St. Louis, MO in 2016, and stratified the analyses by race (non-Hispanic White (NHW) vs. Black/African American). Participants self-reported the number of servings of coffee, caffeinated tea, and decaffeinated tea they consumed. Volpara software was used to determine volumetric percent density (VPD), dense volume (DV), and non-dense volume (NDV). We used generalized linear regression models to quantify the associations of coffee and tea intake with MBD measures. Coffee: ≥1 time/day (ß = 1.06; 95% CI = 0.93-1.21; p-trend = 0.61) and caffeinated tea: ≥1 time/day (ß = 1.01; 95% CI = 0.88-1.17; p-trend = 0.61) were not associated with VPD. Decaffeinated tea (≥1 time/week) was positively associated with VPD in NHW women (ß = 1.22; 95% CI = 1.06-1.39) but not in African American women (ß = 0.93; 95% CI = 0.73-1.17; p-interaction = 0.02). Coffee (≥1 time/day) was positively associated with DV in African American women (ß = 1.52; 95% CI = 1.11-2.07) but not in NHW women (ß = 1.10; 95% CI = 0.95-1.29; p-interaction = 0.02). Our findings do not support associations of coffee and caffeinated tea intake with VPD in premenopausal women. Positive associations of decaffeinated tea with VPD, with suggestions of effect modification by race, require confirmation in larger studies with diverse study populations.

OBJECTIVES: Coffee, green tea, and caffeine are potential preventive factors for dementia, but the underlying evidence is insufficient. This study aimed to examine associations between the consumption of coffee, green tea, and caffeine and dementia risk in middle-aged and older people. METHODS: This was a cohort study with an 8.0-year follow-up. Participants were community-dwelling individuals (n = 13,757) aged 40-74 years. A self-administered questionnaire survey was conducted in 2011-2013. Predictors were the consumption of coffee/green tea, from which caffeine consumption was estimated. The outcome was incident dementia obtained from the long-term care insurance database. Covariates were demographic factors, body mass index, physical activity, energy, smoking, drinking, and disease history. Adjusted hazard ratios (HRs) were calculated using Cox proportional hazards models. HRs were also calculated using a Cox model with delayed entry. RESULTS: The number of dementia cases during the study period was 309. Participants with higher coffee consumption had lower HRs (adjusted p for trend = 0.0014), with the fifth quintile (≥326 ml/day) having a significantly lower HR (0.49, 95% confidence interval [CI]: 0.30-0.79) than the first quintile (<26 ml/day, reference). Similarly, participants with higher caffeine consumption had a significantly lower HR (adjusted p for trend = 0.0004) than the reference. The Cox model with delayed entry yielded similar results. These associations were significant in men, but not in women. Moreover, participants who consumed 2-2.9 cups/day and ≥3 cups/day of coffee had lower HRs (0.69, 95% CI: 0.48-0.98 and 0.53, 95% CI: 0.31-0.89, respectively) than those who consumed 0 cup/day. The association between green tea consumption and reduced dementia risk was significant (adjusted p for trend = 0.0146) only in the 60-69 years age subgroup. CONCLUSIONS: High levels of coffee and caffeine consumption were significantly associated with a reduced dementia risk in a dose-dependent manner, especially in men. Moreover, coffee consumption of ≥3 cups/day was associated with a 50% reduction in dementia risk.

INTRODUCTION: In 2014, the Navajo Nation Healthy Diné Nation Act (HDNA) was passed, combining a 2% tax on foods of 'minimal-to-no-nutritional value' and waiver of 5% sales tax on healthy foods, the first-ever such tax in the U.S. and globally among a sovereign tribal nation. The aim of this study was to measure changes in pricing and food availability in stores on the Navajo Nation following the implementation of the HDNA. METHODS: Store observations were conducted in 2013 and 2019 using the Nutrition Environment Measurement Survey-Stores (NEMS-S) adapted for the Navajo Nation. Observations included store location, type, whether healthy foods or HDNA were promoted, and availability and pricing of fresh fruits and vegetables, canned items, beverages, water, snacks and traditional foods. Differences between 2013 and 2019 and by store type and location were tested. RESULTS: The matched sample included 71 stores (51 in the Navajo Nation and 20 in border towns). In 2019, fresh produce was available in the majority of Navajo stores, with 71% selling at least 3 types of fruit and 65% selling at least 3 types of vegetables. Compared with border town convenience stores, Navajo convenience stores had greater availability of fresh vegetables and comparable availability of fresh fruit in 2019. The average cost per item of fresh fruit decreased by 13% in Navajo stores (from $0.88 to $0.76) and increased in border stores (from $0.63 to $0.73), resulting in comparable prices in Navajo and border stores in 2019. While more Navajo stores offered mutton, blue corn and wild plants in 2019 compared to 2013, these changes were not statistically significant. DISCUSSION: The findings suggest modest improvements in the Navajo store environment and high availability of fruits and vegetables. Navajo stores play an important role in the local food system and provide access to local, healthy foods for individuals living in this rural, tribal community.

Increasing sugar-sweetened beverages (SSB) consumption and associated health impacts warrant health-policy action. We assessed associations of socioeconomic and lifestyle variables with adolescents' consumption of regular soda (RSD), sport (SD), and energy (ED) drinks. Cross-sectional study of 3930 Spanish adolescents (2089 girls, 1841 boys) aged 13-18 years). We compared frequency of consuming each SSB type (European Food Safety Authority questionnaire) with sociodemographic and lifestyle variables (standardized questions). RSD, SD, and ED were consumed at least weekly by 72.7%, 32.3%, and 12.3% of participants, respectively, and more frequently (p < 0.001) by boys, compared to girls. Multivariate ordinal logistic regression showed inverse association between RSD, SD, and ED consumption and parental occupation-based socioeconomic status (p < 0.01). Daily smoking was associated (p < 0.001) with higher ED (OR 3.64, 95% CI 2.39-5.55) and RSD (OR 2.15, 95% CI 1.56-2.97) consumptions. SD intake was associated inversely with smoking (OR 0.60, 95% CI 0.40-0.89, p = 0.012) and directly with physical activity (OR 2.93, 95% CI 2.18-3.95, p < 0.001). School performance was lower among ED (OR 2.14, 95% CI, 1.37-3.35, p = 0.001) and RSD (OR 1.81, 95% CI 1.24-2.64, p = 0.002) consumers, compared to SD. Maleness and low socioeconomic status predicted SSB consumption. Smoking and low school performance were associated with higher ED and RSD intakes.

BACKGROUND: The World Health Organization recognizes universities as an important health-promotion setting, including in healthy food provision. Previous research shows that healthy food retail interventions also need to consider commercial sustainability, including financial outcomes, and should take a holistic approach to consumer experience. OBJECTIVE: Our aim was to determine the health behavior and commercial outcomes of a multicomponent traffic light-based healthy vending policy implemented as one part of a holistic university food policy. The hypothesis was that purchases of less healthy "red" beverages would decrease compared with predicted sales, that purchases of healthier "green" and "amber" alternatives would increase, and that there would be no change in revenue. DESIGN: A quasi-experimental design evaluated a real-world food policy using monthly aggregated sales data to compare pre-intervention (January 2016 to March 2018) and post-intervention period sales (December 2018 to December 2019). PARTICIPANTS/SETTING: Electronic sales data were collected from 51 beverage vending machines across 4 university campuses in Victoria, Australia. INTERVENTION: A multicomponent policy was implemented between April and November 2018. Beverages were classified using a voluntary state government traffic light framework. Policy included display ≤20% red beverages and ≥50% green beverages; machine traffic light labeling; health-promoting machine branding; review of machine placement; and recycled bottle packaging. MAIN OUTCOME MEASURES: Changes in red, amber, and green volume sales, and revenue compared with predicated sales. STATISTICAL ANALYSES PERFORMED: Interrupted time series analysis of sales data compared post-policy sales with predicted sales. RESULTS: In the 13th month post-policy implementation, there was a 93.2% (95% CI +35.9% to +150.5%) increase in total beverage volume sold and an 88.6% (95% CI +39.2% to +138.1%) increase in revenue. There was no change in red beverage volume sold, but increases in green (+120.8%; 95% CI +59.0% to +182.6%) and amber (+223.2%; 95% CI +122.4% to +323.9%) volume sold. CONCLUSIONS: Sustained behavior change and commercial outcomes suggest that holistic vending interventions can effectively promote healthier beverage sales.

Introduction: insufficient and/or unhealthy total fluid intake (TFI), especially in the early stages of life, may have a negative impact on health. Objective: to assess the current patterns of fluid consumption in children and adolescents in Spain, including drinking occasions and locations (e.g., at home or at school), and to compare TFI with adequate intake (AI) of water from fluids as recommended by the European Food Safety Agency (EFSA). Methodology: a Spanish cross-sectional study was performed assessing TFI from all sources of fluid consumption according to drinking occasions during the day and location, using a validated liquid intake 7-day record (Liq.in7). Data collection occurred between April and May, 2018. A sample of 146 (63 % boys) children (4-9 years old) and adolescents (10-17 years old) was included. Parents reported such information when children were under 16 years. Results: a high proportion of children and adolescents did not meet EFSA-derived reference values for fluid intake (73 % and 72 %, respectively). Forty percent of children and about 50 % of adolescents consumed at least one serving of sugar-sweetened beverage (SSB) per day, while about 20 % consumed only one or less servings of water per day. Consumption during the main meals was most important for both children and adolescents (representing 50 % and 54 % of TFI, respectively), and was mainly driven by water (62 %). Consumption at home in children (70 % of TFI) was made of water (47 %). In the same way, at school, water contributed to half intake. However, adolescent girls at school drink more SSBs (41 %) than water (34 %), the former being the most consumed fluid. At other locations, adolescent boys also drink more SSBs (51 %) than either water (29 %) or milk and derivatives (10 %). (AU)


Introducción: la ingesta total de líquidos insuficiente o poco saludable, especialmente en las primeras etapas de la vida, puede tener un impacto negativo sobre la salud. Objetivo: evaluar el patrón actual de consumo de líquidos en niños y adolescentes en España, incluyendo el número de veces y los lugares para beber, y comparar la ingesta total de líquidos con la ingesta adecuada establecida por la Agencia Europea de Seguridad Alimentaria (EFSA). Metodología: estudio transversal que evaluó la ingesta total de líquidos utilizando un registro validado de ingesta de líquidos de 7 días (Liq.in7). Se realizó entre abril y mayo de 2018, incluyendo una muestra de 146 niños de 4 a 9 años y adolescentes de 10 a 17 años españoles (63 % varones). Los padres detallaron dicha información en caso de que los niños fueran menores de 16 años. Resultados: una alta proporción de niños y adolescentes no cumplían con los valores de referencia de la EFSA para la ingesta de líquidos (73 % y 72 %, respectivamente). El 40 % de los niños y aproximadamente el 50 % de los adolescentes consumían al menos una porción (250 ml) de bebidas azucaradas por día, y el 20 % consumían una porción o menos de agua al día. Durante las comidas principales se consumía la mayor cantidad de líquidos tanto en los niños como en los adolescentes (representando el 50 % y el 54 % de la ingesta total de líquidos, respectivamente), principalmente agua (62 %). AIM: To determine the role of sleep quality and caffeinated beverage consumption in the effect of late chronotype on body mass index (BMI). MATERIALS AND METHODS: The study consisted of a total of 661 healthy university students with a mean age of 21.4 ± 1.38 years. Within the scope of the study, The Morningness-Eveningness Questionnaire (MEQ) was used to determine the chronotypes of individuals, the Pittsburgh Sleep Quality Index (PSQI) was used to determine sleep quality, and a 24 h physical activity record was taken to determine physical activity levels. The caffeine intake of the individuals was determined using a food frequency questionnaire for the intake of caffeinated food and beverages. The role of mediators in the effect of late chronotype on BMI was evaluated via multiple mediation analysis. RESULTS: In this study, evening-type individuals had higher daily caffeine intake, worse sleep quality (P < 0.001), and higher BMI (P < 0.05). When the effect of mediators was taken into account, the negative relationship between the chronotype score and BMI was maintained (ß = -0.109, 95% CI (-0.194, -0.021), P < 0.05). Decreased sleep quality had a mediated effect in this relationship [ß = -0.208, 95% CI (-0.275, -0.119)], and caffeinated beverage consumption was found to have no mediator function (ß = 0.040 95% CI (-0.050, 0.108), P > 0.05). CONCLUSION: To conclude, eveningness chronotype had an effect on higher levels of caffeinated beverage consumption, lower sleep quality, and increased BMI. Chronotype, sleep quality, and caffeinated beverage consumption should be taken into account in studies examining the relationship between circadian rhythm and obesity.

BACKGROUND: Food insecurity, a state of not being able to consistently access nutritious food due to financial constraints, has been associated with poor dietary intake among college students. The extent to which campus food resources contribute to this association is unknown. OBJECTIVES: This study examined the association between food insecurity and dietary intake in a sample of undergraduate students with unlimited meal plans and dining hall access at a large, public Midwestern university. DESIGN: The study design is cross-sectional. The data used are baseline data from a broader sugar-sweetened beverage intervention study that were collected using a Qualtrics survey prior to the intervention. PARTICIPANTS/SETTING: The sample consisted of 1033 undergraduate students recruited from 3 dining halls. The data were collected in November 2018. MAIN OUTCOME MEASURES: Food security was assessed using the 6-item Short Form Food Security Survey Module. Dietary intake was assessed using the National Cancer Institute 26-item Dietary Screener Questionnaire and the Beverage Intake Questionnaire-15. STATISTICAL ANALYSES PERFORMED: Generalized linear regression models were used to examine differences in dietary intake by students' food security status, adjusting for students' sociodemographic characteristics. RESULTS: In the sample, 14% of students were food-insecure. After adjusting for sociodemographic variables, food-insecure students reported 9% lower intake of fruits (P = 0.02), 9% lower intake of vegetables (P < 0.001), 10% higher intake of dairy (P = 0.002), 6% higher intake of total added sugars (P = 0.01), 10% higher intake of added sugars from sugar-sweetened beverages (P = 0.01), 4% higher intake of calcium (P = 0.01), and 4% lower intake of fiber (P = 0.01) compared with food-secure students. With respect to beverage intake, food-insecure students had 56% higher intake of total sugar-sweetened beverages (P = 0.002), which was driven by 185% higher intake of energy and sports drinks (P = 0.001), and 121% higher intake of sweetened teas (P = 0.001). CONCLUSIONS: Despite having identical food resources within campus dining halls, there were significant differences in the diets of college students by food security status.

BACKGROUND: There is a dearth of information regarding the association between coffee consumption and its health effects with respect to mortality among Korean people. OBJECTIVE: The aim of this study was to examine the association between coffee consumption and all-cause mortality and cause-specific mortality risks in the Korean population. DESIGN: This prospective cohort study had a median follow-up period of 9.1 years. PARTICIPANTS/SETTING: In total, 173,209 participants aged 40 years and older from the Health Examinees study were enrolled between 2004 and 2013. The analytic sample included 110,920 participants without diabetes, cardiovascular disease (CVD), or cancer at baseline who could be linked with their death information. MAIN OUTCOME MEASURES: Deaths of participants until December 31, 2018 were ascertained using the death certificate database of the National Statistical Office. Cause of death was classified according to the International Classification of Diseases, 10th Revision. STATISTICAL ANALYSES PERFORMED: Participants were categorized according to the amount and type of coffee consumed. Cox proportional hazards regression analysis was performed to estimate the hazard ratio (HR) and 95%CI of all-cause mortality and cause-specific mortality, such as CVD and cancer mortality. RESULTS: Compared with nonconsumers of coffee, participants who consumed > 3 cups/day had a reduced risk of all-cause mortality (HR 0.79, 95% CI 0.66 to 0.95). Participants who consumed ≤1 cup/day and 1 to 3 cups/day had a reduced risk of CVD mortality (≤1 cup/day: HR 0.58, 95% CI 0.69 to 0.94; 1 to 3 cups/day: HR 0.62, 95% CI 0.41 to 0.96). CONCLUSIONS: This study provides evidence that greater coffee consumption is associated with a decreased risk of all-cause mortality and moderate coffee consumption (approximately 3 cups/day) is associated with a decreased risk of CVD mortality, regardless of the type of coffee, in a Korean population.

INTRODUCTION: Introduction: insufficient and/or unhealthy total fluid intake (TFI), especially in the early stages of life, may have a negative impact on health. Objective: to assess the current patterns of fluid consumption in children and adolescents in Spain, including drinking occasions and locations (e.g., at home or at school), and to compare TFI with adequate intake (AI) of water from fluids as recommended by the European Food Safety Agency (EFSA). Methodology: a Spanish cross-sectional study was performed assessing TFI from all sources of fluid consumption according to drinking occasions during the day and location, using a validated liquid intake 7-day record (Liq.in7). Data collection occurred between April and May, 2018. A sample of 146 (63 % boys) children (4-9 years old) and adolescents (10-17 years old) was included. Parents reported such information when children were under 16 years. Results: a high proportion of children and adolescents did not meet EFSA-derived reference values for fluid intake (73 % and 72 %, respectively). Forty percent of children and about 50 % of adolescents consumed at least one serving of sugar-sweetened beverage (SSB) per day, while about 20 % consumed only one or less servings of water per day. Consumption during the main meals was most important for both children and adolescents (representing 50 % and 54 % of TFI, respectively), and was mainly driven by water (62 %). Consumption at home in children (70 % of TFI) was made of water (47 %). In the same way, at school, water contributed to half intake. However, adolescent girls at school drink more SSBs (41 %) than water (34 %), the former being the most consumed fluid. At other locations, adolescent boys also drink more SSBs (51 %) than either water (29 %) or milk and derivatives (10 %). Conclusion: the drinking habits of Spanish young populations are far removed from current recommendations because of a low fluid intake, specifically water, and a high proportion of SSB consumption in children and adolescents. Interventions to ensure that EFSA TFI recommendations are met are of special importance for children and adolescents, with - according to our results - a special focus on male adolescents.


INTRODUCCIÓN: Introducción: la ingesta total de líquidos insuficiente o poco saludable, especialmente en las primeras etapas de la vida, puede tener un impacto negativo sobre la salud. Objetivo: evaluar el patrón actual de consumo de líquidos en niños y adolescentes en España, incluyendo el número de veces y los lugares para beber, y comparar la ingesta total de líquidos con la ingesta adecuada establecida por la Agencia Europea de Seguridad Alimentaria (EFSA). Metodología: estudio transversal que evaluó la ingesta total de líquidos utilizando un registro validado de ingesta de líquidos de 7 días (Liq.in7). Se realizó entre abril y mayo de 2018, incluyendo una muestra de 146 niños de 4 a 9 años y adolescentes de 10 a 17 años españoles (63 % varones). Los padres detallaron dicha información en caso de que los niños fueran menores de 16 años. Resultados: una alta proporción de niños y adolescentes no cumplían con los valores de referencia de la EFSA para la ingesta de líquidos (73 % y 72 %, respectivamente). El 40 % de los niños y aproximadamente el 50 % de los adolescentes consumían al menos una porción (250 ml) de bebidas azucaradas por día, y el 20 % consumían una porción o menos de agua al día. Durante las comidas principales se consumía la mayor cantidad de líquidos tanto en los niños como en los adolescentes (representando el 50 % y el 54 % de la ingesta total de líquidos, respectivamente), principalmente agua (62 %). Es importante que las intervenciones que intenten mejorar la ingesta de líquidos en los niños y adolescentes basen sus objetivos en alcanzar los valores de referencia de la EFSA con un enfoque especial, según nuestros resultados, consistente en mejorar los comportamientos de ingesta de líquidos en los adolescentes varones.

The topic of hydration of children over 3 years of age is often neglected in general pediatric consultations, even though proper hydration is essential for optimal physical and intellectual functions. OBJECTIVE: To evaluate the daily water and beverage consumption of 3- to 8-year-old patients in a private pediatric practice through a retrospective observational study. METHOD: This single-centre study was conducted in a French private practice with five pediatricians and included 200 healthy children, aged 3-8 years. RESULTS: Recommended intake of beverages was reached in 62% of 54 children in the 3- to 4-year-old group and 43% of the 146 children in the 4- to 8-year-old group. Recommended beverages intake of±20% was found in 55% of the 3- to 4-year-old group and 45% of the 4- to 8-year-old group. The potential risk factors of inadequate hydration could not be demonstrated in this study. Water alone was the predominant intake in 63% of the 200 children; milk accounted for 19%, and sweetened drinks for 17%. CONCLUSIONS: These results show a persistent gap between recommended and observed total beverage intake. Although our results are more robust than previous French and international published data, they remain insufficient. In the future, a systematic approach to hydration with parents and children is needed including personalised advice, while maintaining other public health interventions in schools and canteens.

BACKGROUND: We examined the role of post-diagnostic coffee and tea consumption in relation to breast cancer-specific and all-cause mortality among women with breast cancer in prospective cohort studies. METHODS: We identified 8900 women with stage I-III breast cancer from 1980 through 2010 in the Nurses' Health Study (NHS) and from 1991 through 2011 in the NHSII. Post-diagnostic coffee and tea consumption was assessed by a validated food frequency questionnaire every 4 years after diagnosis. RESULTS: During up to 30 years of follow-up, we documented 1054 breast cancer-specific deaths and 2501 total deaths. Higher post-diagnostic coffee consumption was associated with a lower breast cancer-specific mortality: compared with non-drinkers, >3 cups/day of coffee was associated with a 25% lower risk (hazard ratio (HR) = 0.75, 95% confidence interval (CI) = 0.59-0.96; Ptrend = 0.002). We also observed a lower all-cause mortality with coffee consumption: compared with non-drinkers, >2 to 3 cups/day was associated with a 24% lower risk (HR = 0.76, 95% CI = 0.66-0.87) and >3 cups/day was associated with a 26% lower risk (HR = 0.74, 95% CI = 0.63-0.87, Ptrend < 0.0001). Post-diagnostic tea consumption was associated with a lower all-cause mortality: compared with non-drinkers, >3 cups/day was associated with a 26% lower risk (HR = 0.74, 95% CI = 0.58-0.95; Ptrend = 0.04). CONCLUSIONS: Among breast cancer survivors, higher post-diagnostic coffee consumption was associated with better breast cancer and overall survival. Higher post-diagnostic tea consumption may be related to better overall survival.

OBJECTIVE: To demonstrate how the mechanism and agent of injury can influence the anatomical location of a scald. DESIGN: Prospective multicentre cross-sectional study. SETTING: 20 hospital sites across England and Wales including emergency departments, minor injury units and regional burns units. PATIENTS: Children aged 5 years and younger who attended hospital with a scald. MAIN OUTCOME MEASURES: Primary outcome: a descriptive analysis of the mechanism, agent and anatomical location of accidental scalds. Secondary outcome: a comparison of these factors between children with and without child protection (CP) referral. RESULTS: Of 1041 cases of accidental scalds, the most common narrative leading to this injury was a cup or mug of hot beverage being pulled down and scalding the head or trunk (132/1041; 32.9% of cases). Accidental scalds in baths/showers were rare (1.4% of cases). Accidental immersion injuries were mainly distributed on hands and feet (76.7%). There were differences in the presentation between children with accidental scalds and the 103 who were referred for CP assessment; children with scalds caused by hot water in baths/showers were more likely to get referred for CP assessment (p<0.0001), as were those with symmetrically distributed (p<0.0001) and unwitnessed (p=0.007) scalds. CONCLUSIONS: An understanding of the distributions of scalds and its relationship to different mechanisms of injury and causative agents will help clinicians assess scalds in young children, particularly those new to the emergency department who may be unfamiliar with expected scald patterns or with the importance of using appropriate terminology when describing scalds.

In 2019, Canada's Food Guide (CFG) was updated from the 2007 version. This study developed a food-based nutrient profile model (NPM) to evaluate the alignment of packaged food and beverage products with CFG 2019 and compared it with CFG 2007. Packaged products from the University of Toronto's Food Label Information Program 2017 database were evaluated in terms of their alignment with CFG 2007 (using the Health Canada Surveillance Tool (HCST)) and CFG 2019 (using our newly developed CFG 2019 NPM). Agreement in alignment (e.g., products "in line" according to CFG 2019 NPM and in Tiers 1 or 2 according to the HCST) was calculated and differences in alignment and reasons for differences were quantified and described. Overall agreement in product alignment between CFG 2007 and 2019 was 81.9%, with fewer products aligned with CFG 2019: 16.4% vs. 31.8%, (χ2 = 189.12, p < 0.001). Differences in alignment varied across food categories (0.0-73.8%), explained by differences in CFG 2019, reflected in the CFG 2019 NPM (e.g., emphasis on avoiding processed foods, encouraging whole grains and low-fat dairy). This study presents a first step in assessing packaged foods' alignment with CFG 2019; future work is needed to evaluate broader dietary adherence to the updated recommendations. Novelty: A food-based nutrient profile model was developed based on the 2019 CFG and tested on packaged foods by comparing it with the nutrient-based HCST, based on CFG 2007. Most (82%) packaged products were "not in line" with either CFG version.

Caffeine consumption has been linked to poor sleep health in adolescents, but it is unknown whether poor sleep predicts caffeine consumption, and/or whether caffeine consumption predicts poor sleep, particularly when sleep is measured objectively. Data were collected from a micro-longitudinal sub-study of the age 15 wave of the Fragile Families and Child Wellbeing Study (n = 589). Adolescents wore an actigraphy device and completed daily surveys for ~1 week. Daily surveys assessed subjective sleep quality and caffeinated beverage consumption (0 = no caffeine, 1 = any caffeine). Separate mixed models assessed whether actigraphy-measured sleep duration, timing, maintenance efficiency, and subjective quality predicted next-day caffeinated beverage consumption within and between adolescents. Variability (standard deviation) of sleep duration and timing, sleep regularity index, and social jetlag were tested as additional between-person predictors. Lagged models tested whether daily caffeinated beverage consumption predicted sleep that night (n = 458). Adolescents with more variable sleep duration and midpoint had higher average odds of consuming caffeinated beverages compared to others. After adolescents consumed ≥1 caffeinated beverage, they had later sleep onset that night and wake time the next morning than usual versus when they did not consume caffeine. Curbing caffeinated beverage consumption may aid in the maintenance of regular sleep schedules and advance sleep timing in adolescents.

The package label is an important factor to be evaluated, for constituting the first contact of the product with the consumer, which may exert influence on the purchase intention. Thus, this study identified and evaluated the importance of front label attributes of a new functional beverage made from cashew nut added with mango juice on consumers' purchase intention; it also evaluated the influence of consumers' attitudinal issues regarding functional foods on the importance of these attributes to the attitude of buying the beverage. Focus group methodology was used to assess which front label attributes of the product are important in the choosing process. The combination of attributes and their levels was used to develop 18 labels, which were analyzed by consumers in relation to their buying intention. The results were analyzed by conjoint and cluster analysis. The most relevant qualitative information on the purchase intention process of the cashew nut beverage highlighted by the focus groups were the illustration, the nutritional information (0% lactose and 0% cholesterol), and the term "prebiotic" with its functional claim. According to the conjoint analysis, the illustration was the attribute with more influence on the consumers' purchase intention concerning this product, revealing the consumers' preference for the image of the cashew nut standing alone on the label. Nutritional information and the term prebiotic and its functional claim had a positive impact, but were considered of low importance. Regarding the attitudinal factors involving functional foods there was no significant difference among the three groups of consumers.
